Role of operative treatment in current therapy for intractable epilepsies
In spite of the introduction of new antiepileptic drugs, there remain a minor proportion of cases where medicament therapy does not produce satisfactory results. Owing to technical development of diagnostic and microsurgical methods, these patients can be quite well managed by operative treatment. The most important factors of successful outcome include careful selection of surgery candidates (1-2/100, 000 population per year, proper patient information on the expected operative hazards and benefits, and appropriate choice of the type of procedure and surgery planning for each individual patient. There are three main types of procedure: focus removal, focus disconnection, and combination of the two. In this paper, results of focus removal (ablative procedures) are presented.
