Recurrent herpes genitalis: suppressive vs episodic treatment
AIM: In the present study, we have investigated the efficacy and safety of suppressive acyclovir therapy compared with episodic acyclovir treatment in preventing the recurrent genital herpes infections. PATIENTS AND METHODS: Twenty-two patients (9 males, 13 females), aged 18 years or older, having 4 or more recurrences/year were enrolled in this open comparative parallel-group study. Twelve patients were treated episodically with 1000 mg acyclovir 5 times daily for a 5 consecutive days at the first sign of the disease. Ten patients received suppressive therapy with 400 mg acyclovir twice daily for one year. The patients were followed up for 2 years. RESULTS: Suppressive acyclovir therapy reduced the number of recurrences/year from 6.1 to 1.1 as well as shortened the clinical course of the disease. Episodic acyclovir treatment also shortened the course of the disease but only slightly reduced the number of recurrences from 4.9 to 3.8 recurrences/year. CONCLUSIONS: Suppressive yearlong acyclovir therapy (400 mg twice daily) is an effective and well tolerated treatment for the suppression of recurrent genital HSV infections. It significantly reduced the recurrence rate and shortened the duration of the clinical course of the disease. Suppressive yearlong acyclovir therapy is more efficient than episodic treatment in preventing the recurrent genital HSV infections. Psychological well-being over all has improved for patients suffering more than 4 recurrences/year with suppressive yearlong acyclovir therapy.
