AIM: Analysis of the polymorphism of HLA class II alleles and the Caucasian extended haplotype Cw*0602-B57-DRB1*0701-DQA1*0201-DQB1*0201 in type I and type II psoriasis in the Croatian population. PATIENTS AND METHODS: Seventy-seven unrelated patients with chronic stable psoriasis from the Department of Dermatology University Hospital Rijeka were grouped to type I and type II psoriasis according to the anamnestic data. Analysis of HLA class II alleles were performed by PCR method using sequence specific oligonucleotide probes. RESULTS: Patients with type I psoriasis (early onset, positive family history) showed significant association with DRB1*0701  relative risk (RR) = 5.83 , DQA1*0201 (RR=6.12), DQB1*0201 (RR=3.29) and DQB1*0303 alleles (RR=7.51). Type II psoriatics (late onset, no family history) did not show association with any class II alleles. In type I patients we found significant increase of the Caucasian extended haplotype Cw*0602-B57-DRB1*0701-DQA1*0201-DQB1*0201 in comparison to controls (RR=7.72). This haplotype was not detected in any type II psoriatics. CONCLUSIONS: Type I psoriasis is associated with Cw*0602-B57-DRB1*0701-DQA1*0201-DQB1*0201 extended haplotype in Croatians. Furthermore, DRB1*0701, DQA1*0201, DQB1*0201 and DQB1*0303 alleles carry the highest risk for developing type I psoriasis in our population.
