Selenium, zinc and copper in Croatian elderly with and without metabolic syndrome: Comparative population study

Studied sample comprises "fourth wave" of longitudinal study of chronic diseases (years held 1969, 1972, 1982, and 2006/07 - fourth wave) in six regions in Croatia. Participants were residents of two predominantly rural areas: Adriatic island Vis (VA region) and Virovitica -town and surroundings (RV region ; continental region in Croatia near Hungarian border). In both regions, former survival analysis due to cardiovascular mortality was not statistically homogenous. Better results were obtained in VA region, a Mediterranean area with specific life style and dietary habits that differs from those in continental VR region. In the present study participated mobile, non-institutionalised volunteers, above 70 years of age: 36 subjects from VA and 62 from RV region. Medical check-up and diagnoses of chronic diseases were standardized according to the 9th International Classification of Diseases and Causes of Death, and standardized questionnaire, adapted for data processing from early beginning of the follow-ups in 1969, was used. The study was performed in accordance with ethical principles of the Helsinki Declaration, and officially approved by the Institute's Ethical Committee. Metabolic syndrome was assessed according to International Diabetic Federation (IDF) criteria, that is, waist circumference disturbances plus two of three following findings: high serum glucose concentration, previously diagnosed diabetes mellitus, and anti-diabetic therapy, as well as high triglyceride concentration and increased systolic and/or diastolic blood pressure. Number of volunteers with signs of metabolic syndrome determined by IDF criteria was 29 in VA region and 32 in RV region. Trace element concentrations in sera were determined by means of atomic absorption spectrometry, either electrothermal AAS (for selenium) or flame AAS (for zinc and copper analysis). Accuracy of the method was checked by standard reference material and through regular participation in interlaboratory quality control programs. Glutathione peroxidase (GPx) activity was measured in whole blood by the European standard method. Statistically determined, sample of subjects in the study was homogenous by gender and age. Higher metabolic syndrome prevalence was found in VA region. No statistical homogeneity was found between VA and RV region in eating and drinking habits (potato or pasta or rise consumption ; intake of fish and red vine). In subjects from RV region positive correlation of both serum zinc and copper with total serum cholesterol was found. In subjects of both VA and RV regions copper serum positively correlated with C reactive protein concentration. There were statistically significant differences in values of serum selenium and GPx activity between VA and RV regions, as well as between moderately and highly obese subjects and subjects with diagnosed metabolic syndrome. To conclude, results obtained so far add to data in the field of medical geography. The role of antioxidant indicators during healthy ageing, particularly in elderly subjects with signs of metabolic syndrome, as well as influence of antioxidants in nutrition of mobile elderly population should be further explored.
