Peripheral CD4+CD25+FOXP3+ lymphocytes and IFN-γ genotype in kidney graft recipients : risk for rejection?

Interferon-γ as Th1 cytokine has detrimental role in acute allograft rejection, while recent data suggest that CD4+CD25+ FOXP3+ lymphocytes might be responsible for allograft tolerance. Study attempted to explore if certain IFN-γ production level, as presumed by genotype, was associated with transplant rejection and with proportion of peripheral CD4+CD25+FOXP3+ lymphocytes. Fifty three patients (age 50± 12 years, 23 men) with stable cadaveric kidney graft function (creatinine clearance 68± 27 ml/min) for more than a year (median 4 years, min. 1, max. 22) were included. Interferon-γ genotype was determined using PCR-SSP method. Peripheral mononuclear leukocytes (PMNL) were fenotyped by flow cytometry. History of acute rejection episodes was evidenced based on the clinical and/or biopsy data. There were 12 patients with TT genotype (high producers), 4 of them had history of acute allograft rejection. Twentynine patients were intermediate producers (genotype AT) and 16 of them experienced acute rejection. There was not any acute rejection event among 12 patients with so called low IFN- γ genotype. Proportion of CD4+CD25+FOXP3+ lymphocytes in the population of peripheral mononuclear leucocytes was 2.06± 0.65 for the whole examined group. In TT, AT and AA IFN- γ genotypes there were median 1.89 % (min. 1.62, max. 3.33), 2.12± 0.75 % and 1.9± 0.54 % of those cells, respectively, and no statistical difference was found (F=0.446, p=0.643). There was neither statistical difference in the proportion of CD4+CD25+FOXP3+ lymphocytes between rejectors and non-rejectors (2.16± 0.76, 2± 0.58, ANOVA t=0.848, p=0.4). Those with genotype associated with low IFN-γ production were free of rejection episodes. Proportion of CD4+CD25+FOXP3+ lymphocytes did not differ between the patients with different IFN-γ genotypes and was not associated with history of acute allograft rejection. Low producers IFN-γ genotype might be protective against kidney allograft rejection, but that relation was not followed by distinct proportion of CD4+CD25+FOXP3+ lymphocytes, which were presumed as possible tolerance effectors.
