H. pylori Infection As a Cause of Iron Deficiency Anemia: Facts or Fictions
Aim. The purpose of this study was to evaluate the evidence of the pathogenic role of H. pylori (HP) infection in patients with unexplained iron deficiency anemia (IDA).Patients and methods. A total of 60 patients (48F/12M) with HP infection and IDA were included in the study. All patients had upper gastroduodenal endoscopy and gastroduodenal pathology was identified. HP infection was confirmed by gastric histology. The patients had also colonoscopy and small bowell radiographic examination. Female patients also underwent gynecological status. The patients were divided into two groups. Group A- 30 patients treated with eradication therapy for HP infection without iron replacement therapy. Group B- 30 patients treated with iron replacement therapy only. The both group were comparable in age, sex, BMI, and oral iron supplementation bevor endoscopy. The effect variable were dinamic changes after 30 days in: hemoglobin (Hb) level, mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH), serum iron (SI) and serum ferritin (SF). Results. The median increase in Hb level was higher in Group A 25/30 (83%), than in group B 12/30(40%). p < .001. The level of MVC were higher in group A 23/30 (73%) than in group B 13/30 (43%) p < .001. MCH was lower in group B 11/30(37%) than in group A 21/30 (70%) p < .001. SI was higher in group A 25/30 (83%) than in group B12/30 (40%) p < .001. SF was lower in group B 11/30 (36%) than in group A 21/30 (70%) p < .001. Student T-test, Friedman and Wilcoxon Signed Ranks test were used. Conclusion. According to uor resulsts were found that complete recovery of IDA can be achieved with HP eradication therapy without iron replacement therapy in patients with Hp infection.
