Background: Renal involvement is common in a small vessel vasculitis and could be dominant picture of disease. ANCA associated small vessel vasculitis (AASV) is the most frequent kind of vasculitides in adults. Objective: To present clinical-laboratory features and histopathological characteristics of our 25 patients with renal involvement due to small vessel vasculitides. Patients and Methods: In this retrospecitve study (from 2005. to 2008.) we present 25 patients with clinical-laboratory features of renal disorder, who underwent renal biopsy followed by histo-logical tissue specimens examination (light, imunoflourescence, electronic microscopy). Indications for renal biopsy were nephritic syndrome, nephrotic-nephritic syndrome, nephrotic syndrome in 14, 10, 1 patients, respectively. In all patients serum creatinine level, 24h proteinuria, urine sediment, serum ANCA level (ELISA) and VDI (Vasculitis Damage Index) were determinated. Results: This retrospective study included 25 patients (15 male and 10 female) average age of 54 years (from 19 to 79) with histological evidence of pauci-immune focal or diffuse necrotising cresentic glomerulonephritis. Twenty patients had nephritic urine sediment and four had oliguria. Average 24h proteinuria was 3, 54 g and an average serum creatinine value of 352 &micro ; ; ; ; ; mol/L. Average CRP level was 22 mg/L. pANCA was positive in 8 patients, cANCA in 2 and cANCA+pANCA in 1. Microscopic polyangiitis (MP) was confirmed in 12 patients, renal limited MP in 10 and Wegener granulomatosis (WG) in 3 patients. VDI score ranged from 3 to 8. Conclusion: In this study we present 12 patients with microscopic polyangiitis, 10 with renal limited MP and 3 with WG. pANCA was positive in 8 patients (32%), cANCA in 2 (8%) and cANCA+pANCA in 1 (4%).
