Introduction: The degree of dopaminergic neurons loss in Parkinson's disease (PD) can be evaluated in vivo by neuroimaging technique using a radiopharmaceutical that selectively binds to cells transporting neurotransmitter dopamine (dopamine transporter - DAT). One of the most used radiopharmaceutical for DAT scanning by SPECT (single-photon emission computerized tomography) technique is 123I-FP-CIT (123I-Ioflupane). Aim: Evaluation of the clinical value of brain SPECT imaging with 123I-Ioflupane in precise and early discrimination between an essential tremor (ET) without degeneration of the nigrostriatal pathway and PD, with a view to purpose of an adequate and early treatment of patients with PD. Patients and methods: We investigated DaTSCAN binding in 27 patients with clinically uncertain parkinsonian syndromes (12 male, 15 female ; mean age 60 ± 16 years). An accumulation of radiopharmaceutical in presynaptic terminals of the dopaminergic neurons in the striatum, shown by the brain SPECT with 123I-loflupane, was interpreted by a visual method and evaluated by region of interest technique. Results: The presynaptic degeneration of dopaminergic neurons was found in 14 patients, while 13 patients with ET had a regular distribution of presynaptic dopaminergic neurons in the basal ganglias. The ratio, normalized by semiquantitative analysis, between striatum and the occipital cortex was interpreted in a combination with the visual evaluation. Conclusion: DaTSCAN, as a marker of nigrostriatal neuronal integrity in vivo, allows an accurate clinical diagnosis of PD, especially at the early stages of the disease. Apparently, it is an important addition in the clinical practice which enables an early introduction of neuroprotective therapy.
