Young onset multiple sclerosis in Croatia : experience from Gorski Kotar and Istria

Background: Childhood MS (including or before 15th birthday) and young onset MS (including or before 21th birthday) are infrequent. In Gorski kotar, a high risk area for MS in Croatia this disease onset at an earlier age while do not in the surrounding areas characterised by lower prevalence and/or incidence rates. Settings: MS patients from Gorski kotar (30,545 inhabitants in 1991) and nort east Istria )52,428 inhabitants in 1991). Objective: To asses the frequency of childhood and young onset MS among patients from Gorski kotar and north east Istria and to analise the clinical presentation and course in MS patients from Gorski kotar. Methods: The ascertainment was performed on a full 50-year period, from 1948 to 1999 and all available case sources were screened. The clinical data of MS patients were recorded in the European database for multiple sclerosis (EDMUS). Verified, clinically definite and laboratory supported definite cases of MS, classified according to the Poser criteria, were considered. The analysis was done for Gorans (born in Gorski kotar) with onset of MS in Gorski kotar, resident population from Gorski kotar (Gorans + born elsewhere) and Gorans with onset elsewhere. In north east Istria the analysis was done on resident population alone. Results: 67 MS patients from Gorski kotar and 39 from north east Istria fulfilled the criteria. Gorans with onset of MS in Gorski kotar (50) had an earlier mean age at onset (26.0 +/- SD 8.3) than those with elsewhere onset of MS (30 +/- SD 7.9) or those born elsewhere (27.3 +/- SD 8.9). Among Gorans with onset in Gorski kotar 17 were young onset MS (34%) and four them were childhood MS (8%). Two of the Gorans with onset elewhere (10) and two of the born elsewhere (7) were young onset MS. In north east Istria the mean age at onset of MS was 31.1 +/- SD 7,6. Only three (7.7%) patients were young onset MS (tho of them childhood onset MS). All patients with onset of MS in Gorski kotar had remittend (5) or remittent progressive course (14). Fourteen (73.7%) of them had a monosymptomatic presentation: motor 5, optic neuritis 4, sensory 2, brainstem 2, and cerebellar 1. Conclusions: Early onset of MS in Gorski kotar may be partially conditioned by the environment.
