Parameters of oxidative stress in urine of residents from the area with endemic nephropathy in Croatia
Endemic nephropathy (EN) is a human chronic tubulointerstitial renal disease that occurs in rural areas of Bosnia and Herzegovina, Bulgaria, Croatia, Serbia and Romania. The disease has insidious onset, with fatal outcome affecting persons mostly in the sixth and seventh decade of life (Pleština 1992). Epidemiologic survey from spring 2005 in Croatia demonstrated that the prevalence of EN is the same (0.6-2.3%) as in past decades making this disease still serious health problem (Miletić-Medved et al. 2007). In EN area, first in Bulgaria and later on in other countries, unusually high frequency of otherwise rare upper urinary tract tumors (UTT) were noticed (Puchlev et al. 1960). These tumors are usually multiple and malignant localized mostly bilaterally in the renal pelvis, while the localization in the urinary bladder is not higher than in other regions. In the period 1995-2002 specific mortality of all patients with UTT in EN area in Croatia was 14 times higher than in the rest of the region with EN area and 55 times higher than overall in Croatia (Miletić-Medved et al. 2005). Since extensive production of reactive oxygen species leading to oxidative stress has been implicated in tumor development the aim of this study was to see is oxidative stress involved in development of EN and UTT. Urine samples (N=22) from EN village Kaniža and from control village (N=16) were collected and analyzed for presence of product of lipid peroxidation, malondialdehyde (MDA) and product of oxidative damaged DNA, 8-hydroxy-2’deoxyguanosine (8-oxodG). Level of both parameters of oxidative stress, MDA and 8-oxodG, were significantly higher in EN village than in non-EN village. These results indicate that oxidative stress could be implicated in development of EN and UTT.
