Maxillary fragment stabilisation after Le Fort I fracture with one screw per fragment
Purpose The aim of the present study was to determine if plates with only one screw pair can be used for Le Fort I fracture management. Good postoperative results motivated direct application of mandible fixation principles to the fractured midface region without the additional experimental research. However, amount and distribution of the forces in the midface region is different than on the mandible. Materials and Methods Testing was conducted on plastic anatomical models. Validity of experimental model was tested before fixation techniques were compared. Standard mini plates and mini screws were used for fixation of the maxilla. The model surface strain analysis was conducted by non-contact object grating method that enabled the surface strain measurement without direct influence on the measured model. Results In two screw pair fixation outer screw pair has little effect on local strain distribution, but it lowers contact forces along the crack. One screw pair fixation is stable enough for fixation but it has a higher strain peak at the crack edges. Conclusions One screw pair per plate is enough for stable fixation, and two or more screw pairs should be only used when bone fragment at the fracture site can not sufficiently transmit forces along the crack.
