What is the future of sinus surgery?
Nowadays, a standard method of the sinus surgery is functional endoscopic sinus surgery – FESS. This method succeeded radical sinus surgery which was characterized by a devastating approach with no respect of sinonasal mucosa. FESS was the first surgical model to address underlying pathophysiologic mechanisms of chronic sinusitis. Characteristics of FESS are minimal invasiveness, maximally exact surgery, elegant and efficient surgery. It was called surgery without surgery, and soft surgery. The idea of being as conservative as possible in surgical treatment of chronic sinusitis, and also in the treatment of neoplasms of the nose and paranasal sinuses is fully justified by favorable surgical outcomes. To be frank, efficiency of functional endoscopic sinus surgery is not significantly better than efficiency of the radical sinus surgery. So, what is the future of sinus surgery? The question is whether philosophy and method of „original“ FESS have been changing during the last decade? Does a new technique, such minimal invasive sinus technique (MIST) or baloon sinuplasty offer any new approach, or just reveal something what FESS in its original technique offered from the very beginning of this method? Where is the place of computer-assisted surgery in sinus surgery? On the other hand, the question is whether FESS has been escaping from otolaryngology to neurosurgery. Has FESS moved from its natural specialty and become a method which improved neurosurgery? Has FESS just become the tool to achieve neurosurgeon procedure with an otolaryngologic approach. So, do we need a new philosophy, a new method and technique in sinus surgery, and what it will be like, or FESS is the best solution ever?
