Selective and nonselective beta blockers in primary open angle glaucoma therapy - results of color Doppler sonography

Primary open angle glaucoma (POAG) is a syndrom of progressive optic neuropathy with optic nerve head excavation and visual field defects. Poor correlation between IOP and progression of glaucoma dissease put vascular mechanism in the center of attention. By Color Doppler sonography, kvantification of changes in blood vessels which supply optic nerve head is posssible. AIM: To find is there any changes in the circulation of central retinal artery and posterior ciliary arteries in patients with primary open angle glaucoma treated by selective or nonselective b- blockers. METHODS: 44 patients (88 eyes) were divided into two groups: Group 1: 22 patients (44 eyes) under selective b- blockers therapy (Betaxolol 0,5%), and Group 2: 22 patients (44 eyes) under nonselective b- blockers therapy (Timolol 0,5%). Vascular indices (RI, PI) were measured in central retinal artery and posterior ciliary arteries. RESULTS: 1) decreased blood flow and increased vascular indices in both group of patients, 2) statistically significant difference between group 1 and group 2: blood flow is higher and vascular indices are lower in group 1 (Betaxolol 0,5%) compare to group 2 (Timolol 0,5%). CONCLUSION: Selective b- blockers (calcium channel blockers) acts more vasoactively and neuroprotectively comparing to nonselective b- blockers.
