Hemodinamical and clinical role of occluded vertebral artery
The incidence of occluded vertebral arteries(VA) in 140 stroke patients treated in departement of neurology in one year period was evaluated. The criteria for extracranial occlusion were positive vesselidentification, absent flow, presence of collateral flow. Brain computer tomography CT), color coded Doppler of carotid and vertebral arteries qwere performedin all patients. Risk factors were assesed by lipidogram, blood glucose and presence of hypertensia and cardiovascular diseases. In the group of 140 patients, the occlusion of VA in extracranial segment was found in 19 patients ( 15 males and 4 females). The mean age was 66, 8 years. Risk factors were pathological lipidogram(36, 8), hypertension(63%), and diabetes mellitus 26 %.Seven patients with occlusion of one VA, had concomitant significant stenosis of internal carotid artery(3 ptients had subtotal stenosis of left and 1 of right ACI, one had 75% stenosis of left ACI, while in one case we found occlusion of left ACI), and the rest of patients had normal carotid arteries. Brain CT findingas: ischemia of arterial territory(42%), cortical athrophy(31%9, and multiple lesions(26%). No normal brain CT was found. The present study pointed to the importance of vertebral arteries in pathophisiology of stroke, also the frequent findings of occluded VA together with significant stenosis of ACI.
