Relationship between albuminuria and thyroid function in euthyroid patients with type 1 diabetes mellitus
In this study of a large cohort of patients with type 1 diabetes we provided the evidence that albuminuria in euthyroid patients with type 1 diabetes, with normal serum creatinine levels and creatinine clearance, was associated with decreased serum thyroid hormone and especially TSH levels. The decreased thyroid function in patients with non-thyroidal diseases like chronic kidney disease is referred to as the euthyroid sick syndrome. Thyrotropin secretion is decreased, which causes decreased thyroidal secretion of fT3 and decreased availability of T4 for peripheral conversion to T3. The decrease in TSH secretion, despite lower levels of circulating thyroid hormone, explains the euthyroid sick syndrome as a host's defense mechanism against protein wasting. We showed that these abnormalities already occurring in patients with albuminuria who had normal thyroid status, serum creatinine and creatinine clearance.
