Sonographic features of craniocervical artery dissection
Craniocervical artery (CCA) dissection are primary diagnosed by angiography, MR or MRA. Sonographic findings include a variety of findings, and frequently are missed due to localisation. The aim of the study was to analyse the spectrum of sonographic findings in patients, who presented with clinical or ultrasonographic dissection of CCAs investigated in Neurology Doppler laboratory during two years period. From January 1th 1998 to December 31th 1999. 42 patients with CCA dissection were investigated. 23 patients clinically presented with CCA dissection and in 19 ultrasonography double lumen or bifurcation stenosis with double lumen. Color and power Doppler of carotid and vertebral arteries was performed on Acuson 128XP. Color and power Doppler scans and hemodynamic spectra were analysed. The data are presented descriptively. Out of 42 patients 4 had multivessel involvement. Out of 37 carotid dissections 6 presented with string sign, 1 string and pearl sign, 1 intimal flap, 9 double lumen, 18 bifurcation stenosis with double lumen under the plaque base, and 2 hemodynamic spectra suggesting distal occlusion. Out of 9 vertebral artery dissections 1 had intimal flap, 2 double lumens and 6 hemodynamic spectra suggesting distal occlusion. 2 subclavian arteries showed double lumen. Color and power Doppler sonography are showing a broad spectrum of findings in CCA dissection and thus are useful methods for its screening.
