Anti-inflammatory effects of percutaneous coronary intervention in acute myocardial infarction
The aim was to investigate changes in peripheral blood (PB) leukocyte subsets in patients with ST elevation myocardial infarction (STEMI) underwent primary percutaneous intervention (PCI) and conservatively treated patients with myocardial infarction without ST elevation (NSTEMI) during early rehabilitation. Peripheral blood mononuclear cells were isolated by density centrifugation and lymphocytes were analyzed by flow cytometry. Absolute numbers of leukocytes and platelets were enumerated by hematology analyzer. Troponin I was measured by ELISA. In NSTEMI patients the absolute neutrophil count, lymphocytes and monocytes increased on day 21, when compared with healthy examinees. Trombocytes number increased in NSTEMI patients, but do not reach normal value. In NSTEMI patients a decrease in T cell percentages appeared earlier during the rehabilitation and was followed by a decrease in mean fluorescence intensity (MFI) for CD3. In STEMI patients, T cell drop occurred later and a decrease in MFI for CD3 preceded the changes in T cell frequency. Increased NKT cell frequency, CD56+dim cells proportion, and decreased MFI for CD56 on CD56+bright cells at the beginning of rehabilitation in comparison to the end indicate down-regulation of inflammatory reaction in NSTEMI patients. In STEMI patients, the MFI for CD56 on CD56+bright NK cells decreased. In spite of a lower troponin I concentration, NSTEMI patients treated conservatively have more profound changes in absolute leukocyte and platelets numbers and frequency of lymphocyte subpopulations.
