VARIOUS EXPERIENCE WITH VIBROPLASTY APPLICATION
Vibroplasty is a surgical procedure used to restore social hearing with active middle ear implant – Vibrant Soundbridge (VSB). In the beginning, vibroplasty was performed only in hearing restoration in cases of sensorineural hearing loss. Recently, it is also used to restore hearing in cases of conductive and mixed hearing loss. Hearing restoration with passive implants is often faced with limitations due to tubal-middle-ear dysfunction. Active implant (VSB) directly vibrates the small bones in the middle ear and is successfully used mostly in cases of sensorineural hearing loss. In global experience, a standardized and effective coupling of the floating mass transducer of the VSB is placed to the intact stapes via the mastoidectomy and a posterior tympanotomy and via the large facial recess. Also, various locations and attachments of a Vibrant transducer were investigated – in cases of conductive and mixed hearing loss after tympanoplasty and in tympanosclerosis. Besides eradication of chronic middle ear disease, the reconstruction of the sound conduction apparatus is a major goal of modern ear microsurgery. Experiments with VSB resulted in the development of a new titanium clip holder for a Vibrant integrated total ossicular replacement prosthesis assembly with placement on the footplate (Hüttenbrink KB, Beutner D, Bornitz M, Luers JC, Zahnert T.). Placing a transducer directly on the footplate via a rod transmission gave a better gain for the high frequencies than in the round window location. The acoustic results of the patients showed an improved gain in speech understanding, unobtainable by a conventional hearing aid. Also, in recent investigations, a transducer can be placed to a „thrird window“. The third window was performed anterior inferior to the round window. Preliminary data of this report reveal that vibroplasty with coupling of the FMT directly to a third window leads to similar audiological results compared with the conventional coupling of the FMT on the round window niche (Pau HW, Just T.). Global experience and investigation with the VSB show that a majority of patients reported a significantly improved sound clarity and overall sound quality.
