Treatment of sexually transmitted diseases in travellers

Sexually transmitted diseases are, in adults, among the most common infectious diseases. Today, STD remain important and can be transmitted by heterosexual or homosexual contacts. Risk groups fro bacterial STD are among others persons in mobility as are traveller, tourists, seamen, airline personal. Their rate of infection depends on the number of sexual contacts, indiscrimination in the choice of partners and no use of prophylactic measure. Particular attention is to pay to prostitution, promiscuity in young people and alcohol drinking habits. Aim: epidemiological data about the frequency of STD among traveller is scanty. Treatment of these patients require special attention. For this reason we performed a study on the frequency of STDs in person on the move and travellers. Results: Chlamydia trachomatis infections are the most frequent bacterial STD, causing urethritis in men and cervicitis in women. About 10% of all cases are registered in travellers. Many patients are asymptomatic and this promote the spread of C.trachomatis in the population. The infection can have heavy consequences in women, leading to pelvic inflammatory disease, and infertility. Today the diagnosis is easy and the disease curable with azithromycin as a single oral dose of 1.0. Alternatively can be used doxycycline 2x100mg/day fro 7 days. Gonococal infections are now relatively infrequent, but can also cause a lot of complications. Neisseria gonorrhoeae is becoming more and more resistant to numerous antibiotics. In the treatment ceftriaxon 125mg i.m., ciprofloxacin 500mg as single oral dose, or azithromycin 1.0 fro two days. A single dose of 1.0 g of azithromycin was also effective in the treatment of chancroid and in cases of vaginosis caused by Gardnerella vaginalis. Syphilis also seen rarely, but is relatively frequent in travellers (40% of all cases). Penicillin remain the drug of first choice in all stages of the disease, but in patient allergic to penicillin azithromycin can be used. Conclusion: STD are frequently observed in travellers in our region. The diagnosis and treatment of these patients require special attention. Contacts of the seen patients need also be cured.
