Analysis of cytotoxic T-lymphocytes and natural killer cells in peripheral blood, synovial fluid and synovial tissue of patient with rheumatoid arthritis

Rheumatoid arthritis (RA) is considered to be immunologically mediated disease, with humoral and cellular immunity participating in pathogenesis. The role of humoral immunity has been investigated in some details. Relatively little is known about the presence of natural killer (NK) cells and cytotoxic T lymphoctes (CTL) in this disease. Previous studies of cytotoxic cells in peripheral blood, synovial fluid or synovial tissue from rheumatoid arthritis patients have produced conflicting results. In studies of function and morphology, investigators have reported increased as well as decreased cytotoxicity and numbers of NK cells. Using direct and indirect immunoflorescence we measured the percentage of CD8+, CD16+ and CD56+ cells in peripheral blood (PB), synovial fluid (SF) and synovial membrane (SM) in patients with acute phase of RA. The results were compared with those obtained from patients with inactive RA and osteoarthrosis (OA). In the PB we did not find significant difference in percentages of CD8+, CD16+ and CD56+ cells among all investigated groups. In SF, the highest percentages of CD8+ and CD56+ cells were found in active RA, while the percentage of CD16+ cells was the lowest. In SM, significant increase of CD16+ and CD56+ cells number was obtained in active as well as inactive RA in comparison with OA. Analysing samples in the same group, but from different tissues (intragroup analyses), CD8+ cells were significantly higher in SF in comparison with PB and SM. The number of CD16+ cells were higher in PB in comparison with SF and SM, as well as in SM comparing with SF. Intragroup analysis of CD56+ cells did not show significant difference between PB, SF and SM. The obtained results suggest the existence of increasing cytolytic potential in the joints of patients with active RA (local level), while cytolytic potential on sytemic level (PB) remain unchanged. This result could indicate that cytolytic lymphocytes may play an important role in the immunologically driven destruction of the joint.
