Conservative vs. surgical periodontal therapy

A number of recent discoveries, particularly in the field of etiology and pathogenesis of periodontal disease, have led to a paradigm shift in treatment philosophy. The periodontal treatment basically includes tooth and root debridement, which is not always easy to accomplish because of many problems we encounter, especially in periodontitis patients. Conservative versus surgical periodontal therapy is always a dilemma, and primarily depends on the type of disease, patient attitude, age, financial capabilities and on clinicians’ preferences towards some sort of treatment. Even today a conservative therapy represents the "gold standard" in periodontology. Thanks to the new techniques such as "full mouth disinfection" we have partially reduced the need for surgery, thus resulting in more affordable treatment of advanced periodontitis cases. On the other hand, modern periodontal surgery has evolved, especially in terms of regenerative and mucogingival techniques. The choice of periodontal surgery primarily depends on the type, extent and severity of the disease and finally on the therapist’s skills. Unlike traditional oral surgical procedures, periodontal surgery is microsurgery that requires the acquisition of specific knowledge and skills for which are qualified doctors of dental medicine that are specialists in periodontology. Finally, we cannot forget the great role of dental practitioners who play a primary role in maintaining periodontal health, especially in countries without dental hygienists.
