Prefrontal deficits in the M50 network correlate with neuropsychological function in dementia of the Alzheimer’s type

The amplitude variability of P50/M50 components of electromagnetic responses is commonly used to explore the neural ability to modulate its sensitivity to incoming sensory stimuli in both pathological and healthy aging. The focus of this MEG study was on the dynamic cortical network underlying the M50 complex evoked by a passive auditory oddball paradigm. We used MEG measurements acquired from a 275 channel CTF whole-head system and a multi-dipole, Calibrated Start Spatio Temporal (CSST) localization technique to identify the topology and dynamics of the M50 cortical networks in an effort to discriminate between patients diagnosed with mild cognitive impairment (MCI) or probable AD (n=10) compared to the age-matched controls (n=10, mean=74 age). Binaurally presented stimuli consisted of 400 standard (1.000 Hz) and 100 deviant (1.200 Hz) tones. After neuropsychological testing, evoked magnetic fields were recorded at the Mind Research Network, New Mexico. Multivariate statistical analysis techniques, including PCA and MCDA, were performed for the discernment of categories in the data. Spatio-temporal analyses identified three brain regions active in the 30-100 ms time window: superior temporal gyrus (STG) activity in 20/20 subjects (18/20 bilateral) and prefrontal (PF) activity (4/20 for standards, 11/20 for deviants). The three clusters of subjects identified by the MCDA strongly correlated with the localization of PF generator in the M50 network and the MMSE/REY test scores: a) PF source evoked by both tones, MMSE=30 and REY≥22, b) PF source evoked only with deviants, MMSE=26-29, REY=17-22, c) lack of PF activity, MSE<26 and REY<17. All subjects (9/20) that lacked PF activity for both tones had significantly higher M50 amplitudes of the STG activity and were diagnosed as MCI (4/5) or probable AD (5/5) patients. These findings may have important implications for further clinical research and have the potential to become biomarkers of the neurodegeneration.
