Algorithm for reentry of Croatian blood donors with positive direct antiglobulin test

Blood donors with positive direct antiglobulin test (DAT) are mainly found out through serological crossmatch in Croatian Institute of Transfusion medicine (CITM) or in Croatian hospitals which are supplied with blood units from CITM. Blood units with positive DAT are returned to the CITM. In CITM, we perform DAT at each subsequent blood donation. In case of positive DAT, we release units of fresh frozen plasma and platelet concentrates for clinical treatment. RBC concentrates are discharged. According to CITM algorithm, following two times negative DAT, we do not performed DAT on next donation and all the products from that donation are concordant for clinical usage. During period of donations we observed some blood donors with positive DAT in which results of DAT testing converted to negative. The aim of this study was to find out the exact time period in which positive DAT converted to negative, in majority of blood donors, and to introduce a new algorithm for reentring them into the donor pool. All the data are collected from the database of CITM from 2001 to 2010. We found out 489 blood donors with positive DAT. DAT was performed in polyspecific microcards (Bio-Rad Laboratories, DiaMed GmbH, Cressier FR, Switzerland) according to the package insert. Among 449 blood donors with positive DAT there were 141 (28%) blood donors which were DAT negative for the next two times followed after the donation with positive DAT. 90 (64%) blood donors converted DAT to negative after 2 years, 41 (29%) after 3–5 years and 10 (7%) after five or more years. Due to our results, new algorithm is implemented. Blood donor with two times positive DAT postpones blood donations for the next 2 years. Afterwords, we performed one more DAT testing. If DAT is negative blood donor reentres into the donor pool, but if it is positive he/she will be permanently deferred and asked for visiting his/her general practitioner.
