Neurological complications in cGVHD
Neurologic complications in chronic graft versus host disease (cGVHD) after hematopoietic stem cell transplantation (HSCT) may include peripheral neuropathy (PN). A significant number of cGVHD patients experience painful muscle cramps and neuropathic pain (NeP). PN may cause motor symptoms or sensory changes. When large nerve fibers are dominantly affected the sensory symptoms are usually negative (sensory loss). Small nerve fibers affection usually causes positive sensory symptoms like paresthesia, allodinia or NeP. Electromyoneurography (EMG) is optimal diagnostic procedure for diagnostics of sensorymotor PN dominantly affecting the large nerve fibers (LNF). Quantitative sensory testing (QST) is diagnostic tool for small fiber neuropathy (SFN) assessment. SFN may be cause of NeP. All types of nerve fibers may be affected in cGVHD which may cause various patterns of PN. Together with neurologic examination, EMNG and QST may be valuable diagnostic procedures to evaluate PN in cGVHD.
