Early diagnostics and early intervention in hearing impairment

Auditory experience during the first year of life has a strong impact on the development of perceptual abilities which are a foundation of further language development. Intensive early stimulation with voice and speech is extremely important for children with hearing impairment: this requires early diagnostics. Precise estimation of type, severity, and aetiology of hearing impairment during the first months of life in infants identified by neonate hearing screening is a much more demanding task than in older children or adults. However, this should not be missed as there is evidence on the significance of early intervention. Early intervention means hearing aid prescription and early rehabilitation of hearing, irrespective of severity of impairment. Early intervention in children with profound hearing impairment is often equated solely with early cochlear implantation, while the exceptionally important period of early amplification and evaluation of a child’s aided abilities is neglected. Hearing aid prescription and fitting differ considerably from the same procedures in older children and adults concerning emergency, influence on hearing and speech, characteristics of sound and speech processing, frequency and way of monitoring benefit, and team approach. In this presentation we will discuss the main challenges of early diagnostics and early intervention regarding advantages as well as limitations of diagnostic procedures performed on neonates and infants, specific aetiologies of hearing loss in children, hearing aid prescription and fitting, extended indications for cochlear implantation, and the impressive performance of digital hearing aids.
