The minimally invasive direct restorations & From endo to success direct restorations

Resin-based composite materials since have been introduced at market, showed improvement of they formula. Changes of composite materials are especially concentrated on the reinforcing filler. Inorganic part of material is reduced in size to produce materials that are more easily polished and have greater wear resistance (Ferracane 2011). High wear resistance is particularly important in posterior region. Using nano-sized anorganic paricles, the content of the anorganic part in the material is increased, filler particles are uniformly dispersed in the organic matrix and the space between the particles are reduced which reinforces and protects organic matrix. Materials based on nanotechnology are developed as conventional composite and flowable composite. Recently, a new composite material G-aenial Universal Flo (GC, Tokio, Japan) was introduced on the market. This material has improved physical, mechanical and optical properties which make this material possible to use alone for posterior restorations. The anorganic part of material is based on strontium glass particles with size of 200 nm. This is the smallest size of particles added to flowable composite materials. According to the manufacturer G-aenial Universal Flo is a tixotropic material which stays in place after application, unlike other flowable composites. This characteristic is especially desirable if restoring cervical parts of teeth (figure). Also material is available in broad spectrum of colors. In a light of minimum interevention for restoring teeth which can be used as new material for posterior restorations, is microlaminated GIC with adhesive and bioactive properties enable hard dental tissue preservation and remineralisation. These new materials have shown long-term clinical success, which is supported with scientific data (reference). The main disadvantage of glass- ionomers has been until recently the low mechanical strength, which made them unsuitable for, use in high stress sites, such as occlusal and approximal areas. Equia system (GC, Tokyo, Japan) is a new product, combination of Fuji IX GP Extra and low viscosity nano-filled coat. According to the manufacturer, Equia is the first glass-ionomer material which can be used as a final restoration in posterior region, because of its good mechanical properties and aesthetics. Similar to other glassionomers it bonds chemically to both dentin and enamel and is bioactive, regarding the release and intake of fluor ions. GIC can also be used for other indication like ART, fissure sealing and internal remineralisation. The major concern is still exsisting how to restore endodontically treated teeth. Usually endodontic treatment is preformed on teeth with sever loose of tooth substance. For this purpose clinical can be used fiber reinforced composite which represents combination of organic resin matrix (bis-GMA, TEGDMA and PMMA) which are forming Interpenetrating Polymer Network (IPN), randomly orientated E- glass fibres and inorganic filler particles. On a same technology are based EverStick fibers. An advantage of fibers are better mechanical properties, reliable bonding to hard dental tissues and aesthetic results. EverStick fibers have different indications: direct and indirect bridges in anterior and posterior region, fabrication of inidividual aesthetic posts after endodontic treatment, periodontal splints, splinting of traumatised teeth and fabrication of fixed retainers after finishing the orthodontic treatment. EverStick fibers represent minimally invasive approach in dentistry as no preparation or minimally invasive preparations are sufficient for retention of these fibers.
