What can we achieve with remineralisation in clinical practice?

The concept of Minimal Intervention Dentistry has become more implemented in every day practice because of increasing understanding of the caries process. Demineralized, non-cavitated enamel and dentin can be “healed”, which should switch treatment plan from surgical approach to the treatment of a caries lesion in minimal invasive way. Restorative treatment strategies in cases of initial hard dental tissue demineralisation include application of remineralising agents with high concetration of fluoride, calcium and phosphate ions which are based on CPP-ACP complex. The advantage od such agents is in providing the excat ratio of minerals needed for healing the lesion. In a cases of already cavitated lesions, new materials with adhesive and bioactive properties enable hard dental tissue preservation and remineralisation. These new materials have shown long term clinical success which is supported with scientific data. The main disadvantage of glass ionomers has been until recently the low mechanical strength which made them unsuitable for use in high stress sites, such as occlusal and approximal areas. Equia system (GC, Tokyo, Japan) is a new product, combination of Fuji IX GP Extra and low viscosity nanofilled coat. According to the manufacturer, Equia is the first glass-ionomer material which can be used as a final restoration in posterior region, because of its good mechanical properties and aesthetics. Similar to other glass ionomers it bonds chemically to both dentin and enamel and is bioactive, regarding the release and intake of fluor ions. GIC can also be used for other indication like ART, fissure sealing and internal remineralisation.
