Human Papillomavirus Male Genital Infections: Clinical Variations and the Significance of DNA typing

Human papillomavirus (HPV)-associated genital pathology represents one of the major problems among sexually transmitted diseases (STDs), mostly due to the high recurrence rate, difficult eradication and oncogenic potential. The aim of this study was to determine the frequency of certain HPV types in men with clinically visible, HPV-associated genital lesions (condylomata acuminata, condylomata plana, Buschke-Löwenstein giant condyloma). Biopsy specimens were taken from 100 male patients, aged 1.5 to 68 years, attending the STD Clinic of the Department of Dermatology and Venereology, University Hospital Zagreb and School of Medicine in the period from 1996 to 2000. HPV detection and typing was performed by means of polymerase chain reaction (PCR) and restriction fragment length polymorphism (RFLP). HPV types 6/11 were found in 79 out of 100 patients, while high-risk HPVs in 21 patients (HPV type 16 in 11, HPV type 18 in 6, HPV 31 and 33 in one patient, respectively). High-risk HPV types were significantly (p < 0.01) more often located on the distal part of the skin and mucous membranes of the external genitalia. HPV types were not always consistent with clinical type of HPV-associated genital lesion. Our findings indicate that high-risk HPV types can also be isolated from condylomata acuminata and not only from condylomata plana as shown in the literature. Thus, we consider HPV detecting and typing of male genital HPV-associated lesions as an important part of the diagnostic procedure, treatment and follow-up.
