Clinical evidences on the use of modern Glass Ionomers

Glassionomer cements (GIC) are materials with adhesive and bioactive properties, which enable hard dental tissue preservation and remineralization. The advantages of GIC are adhesion to moist tooth structure and base metals, anticariogenic properties due to the release of fluoride, thermal compatibility with tooth enamel, biocompatibility and low toxicity. The main drawbacks of conventional GIC are susceptibility to fracture and low wear resistance and these disadvantages have limited their use to areas subjected to low masticatory stresses. In order to avoid certain disadvantages of traditional GIC like low mechanical strenght, different modifications of these materials were released on the market. One of these cements is micro- laminated GIC which combined GIC and protective composite based coat. Similar to other glass ionomers, it bonds chemically to both dentin and enamel and is bioactive, due to release and intake of fluoride ions.These new materials have shown long term clinical success, which is supported with scientific data. To improve physical properties, material based on glass hybrid technology (Equia system) was introduced recently. Material consists of highly reactive small glass particles and high-molecular weight polyacrilic acid, which makes cement matrix stronger and chemically more stable. Protective nano-filled coat increases the mechanical properties and provides smooth surface and aesthetic appearance of the final restoration. In addition to in vitro studies which can be indicative of the behaviour of the new GICs, randomised clinical trials are needed to provide information on the long term success of these restorations.
