Assesment of perforin expression in peripheral blood lymphocytes in psoriatic patients during exacerbation of disease

Psoriasis is a skin disease with a significant immunological background characterized both by systemic and local changes. According to some investigations peripheral blood (PB) T lymphocytes show decrease in the number of cells and decreased immune responsiveness to mitogens and alloantigens. In the skin lesions both CD4+ and CD8+ cells with the cytotoxic activity were detected. It has been suggested that cellular interactions and fluctuations in the psoriatic skin of the different cell types emphasizes fluctuating nature of the disease, e.g. exacerbation and remission. The role of lesional psoriatic T cells activated intralesionally is particularly important. There is very few data about the role of lymphocyte (cell) mediated cytotoxicity (CMC) particularly at the molecular level. CMC is mediated by cytotoxic T lymphocytes (CTL), both CD4+ and CD8+, and natural killer (NK) cells. Both CTL and NK cells contain in their granules cytolytic protein perforin (P), which is a mediator in CMC reactions. There is no data about the expression of P in immunocompetent cells in patients with psoriasis, neither in the PB nor in the lesions. The aim of this study was to analyze P expression in various sets and subsets of P+ PBL, known to have different functions. A total of 17 patients with generalized psoriasis vulgaris, from the Department of Dermatology (8 males and 9 females ; age: 23-60) were investigated. All patients were tested early in the phase of exacerbation. Control group consists of 21 healthy subject corresponding to patients group by age and sex. PB lymphocytes (PBL) were tested either by single or double staining, for simultaneous detection of P (intracellular antigen) and cell surface antigens. The cell membrane permeabilization method and flow cytometric analysis was applied. Single staining analyses showed a marked increase of CD3+ and CD8+ lymphocytes (p<0.001) compared to healthy control. There was no difference in the percentages of CD4+ T cells or NK cells, neither CD16+ nor CD56+. The most interesting finding was significant increase (p<0.05) of total P+ cells compared to healthy control (33, 9% vs. 26.6%). This result points to the potential role of cytolytic mechanisms mediated by perforin at least in the exacerbation of disease. Further analyses are planned to identify P expression in T and NK subpopulations and define the role of these subpopulations in the course of disease.
