COMPARISON OF DIFFERENT SORBENT MATERIALS FOR SOLID-PHASE EXTRACTION OF IDB DRUGS AND FOLIC ACID IN HUMAN PLASMA
Therapeutic drug monitoring is generally defined as the clinical laboratory measurement of a drug or its metabolites that, with appropriate medical interpretation, will directly influence drug prescribing procedures. The goal of this process is to individualize therapeutic regimens for optimal patient benefit. Inflammatory bowel disease (IBD) is characterized by chronic and typically recurrent intestinal inflammation, and it includes Crohn’s disease and ulcerative colitis. Many studies have reported that serum folic acid concentrations differ between IBD patients and healthy individuals. Therefore, the regular serum folic acid screening should be performed in IBD patients, with closer monitoring in patients with ileal resection or active disease. For many years, the IBD treatment was limited to aminosalicylates, corticosteroids and antibiotics. Studies suggesting that the pathophysiology of IBD relates to a disregulated, over-active immune response to unkonwn antigen have led to the increasing importance of immunosuppressive drugs. In this work, solid phase extraction sample preparation procedure for simultaneous determination of folic acid and two immunosuppressive drugs (azathioprine and 6- thioguanine) in human plasma followed by LC/MS/MS determination was optimized. Various reversed phase and mix mode solid phase sorbents were tested and low extraction efficiency was obtained. Excelent recovery up to 100% was obtained using a new innovative sorbent Phree Phospholipid Removal by Phenomenex. Due to fast effective way to remove phospholipids and proteins from plasma samples without negatively affecting the recovery of all target analytes, chromatographic separation of three structurally different analytes was achieve on Symmetry (150 x 4.6 mm, 3.5 µm) column in run time of 7 min. The proposed bioanalytical method allows simultaneus determination of plasma levels of two immunosupresants as well as evaluation of folate status in IBD patients.
