Analiza protetskog rada u periodu 2001.-2010.

Cilj. Svrha rada je analizirati strukturu pacijenata u desetogodišnjem razdoblju. Ispitanici i postupci. Retrospektivno je analizirana struktura vlastitih pacijenata na Zavodu za mobilnu protetiku, koji su pregledani i po potrebi liječeni zbog gubitaka zubi i/ili temporomandibularnih poremećaja (TMP) u periodu 2001.-2010. Pacijenti su liječeni u okviru nastave kliničkih vježbi kolegija mobilne protetike i znanstvenog rada s temom TMP. Rezultati. Ukupno je obuhvaćeno 510 pacijenata, od toga 28, 82% muškog spola i prosječne dobi 49, 08±20, 09 godina (raspon 10-39 godina). Glavnina pacijenata dolazila je iz urbanih sredina (78, 82%) i s područja: grada Zagreba (72, 75%) te Zagrebačke županije (11, 37%). Najčešći protetski rad u gornjoj čeljusti bila je totalna proteza (21, 57%), most (12, 35%), ali većina pacijenata je bila bez protetskog rada (54, 51%). U donjoj čeljusti totalnu protezu imalo je 13, 14% pacijenata, zatim podjednaki broj njih s donjom djelomičnom metalnom (8, 43%) ili akrilatnom protezom (7, 25%), mostom (6, 08%), ali glavnina bez protetske opskrbe (62, 75%). Najčešći razlog dolaska na pregled bilo je klasično protetsko zbirnjavanje (37, 45%), TMP (47, 65%) i najmanje bruksizma (15, 29%). Žene su manje dolazile zbog razloga protetskog zbrinjavanja (p<0, 001). Kod 174 pacijenata provedena je inicijalna terapija gornjom (42, 13%) ili donjom (4, 27%) udlagom. Tijekom promatranog perioda opala je prosječna dob pacijenata (2001. 54, 68±19, 30 naspram 2010. 44, 36±19, 20 godina). Udio ženskog spola se povećao: 2001. bilo ih je 63, 49%, a 2010. 88, 89% (p=0.0550). Udio umirovljenika rapidno je opao: 2001. imali su udjel od 53, 97%, a 2010. samo 24, 44% (p<0, 001). Sukladno tome opao je i broj pacijenata s potpunim protezama (sa 30, 16% na 6, 67%) u gornjoj čeljusti (p=0, 0016). S druge strane u donjoj čeljusti veći je udio pacijenat koji uopće nisu trebali protetiku, a nema predominatnog oblika terapije tijekom godinama, ali najviše se radilo totalnih i djelomičnih proteza u ranijim godinama, a kasnije djelomične metalne proteze i mostovi u manjeg broja pacijenata (p<0, 001). Zaključak. Neminovno se većim udjelom pacijenata s TMP mijenja dobna i spolna struktura te je manje potrebna protetska opskrba pacijenata, što ima utjecaja u planiranju praktičnih zadataka u okviru kliničkih vježbi studenta kolegija mobilne protetike. Objective: The aim of this paper was to analyze the structure of patients in the ten year period. Subjects and procedures: The structure of the patients at the Department of Removable Prosthodontics, who were examined and treated if necessary in the period 2001-2010 due to the loss of teeth and / or temporomandibular disorders (TMDs), was retrospectively analyzed. The patients were treated as part of the clinical practicals which were held within the course of removable prosthodontics and related to a scientific paper on TMDs. Results: The study included 510 patients in total, out of which 28.82% were males and their mean age was 49.08±20.09 (range 10-39 years). The majority of patients came from urban areas (78.82%) and from the Zagreb area (72.75%) and the Zagreb County (11.37%). The most common prosthetic appliance in the upper jaw was total prosthesis (21.57%), bridge (12.35%), but most patients were without any prosthetic replacements (54.51%). 13.14% of patients had a complete denture in the mandible, followed by an equal number of patients with lower partial metal denture (8.43%) or acrylic prosthesis (7.25%), bridge (6.08%), but most of them did not wear any prosthetic appliances (62.75%). The most common reason for coming to the examination was to recieve a conventional denture (37.45%), followed by patients with TMDs (47.65%) and only 15.29% of patients were with a clinical diagnosis of bruxism. Women were less likely to come for prosthetic rehabilitation (p<0.001). The initial therapy was performed in 174 patients with the upper (42.13%) or lower (4.27%) occlusal splint. During the period of observation, the average age of patients dropped (2001: 54.68±19.30 vs. 2010 44.36±19.20 years). The share of female gender increased by 63.49% in 2001, and by 88.89% in 2010 (p=0.0550). The share of the retired patients dropped rapidly: in 2001 they had a share of 53.97%, and in 2010 the share was only 24.44% (p<0.001). Accordingly, the number of patients with complete dentures (from 30.16% to 6.67%) in the maxilla (p=0.0016) has also fallen. On the other hand, regarding the mandible, there was a larger proportion of patients who did not need prosthetics at all and had no predominant form of therapy over the years. The largest number of patients wore the complete and partial dentures in earlier years, and later on a smaller number of patients (p<0.001) wore metal partial dentures and bridges. Conclusion: Inevitably, with a higher proportion of patients with TMDs their age and gender structure changes and the need for prosthetic rehabilitation of patients is less necessary, which has an impact on the planning of practical tasks within the course of removable prosthetic and clinical practicals for dental students.
