The apperance of delusional depression after surgery for colon cancer – a case report

Introduction The aim of this paper is to report a patient who developed a delusional depressive episode after surgery for colon cancer and to demonstrate the significant risk of developing depression and other psychological reactions when faced with the diagnosis and treatment of malignant disease. Case description This is a case of a 69 year old patient who was well and in good physical condition without any history of psychological problems. After finding blood in his stool he was diagnosed with adenocarcinoma of the rectosigmoid colon. He underwent an explorative laparotomy and resection of his rectosigmoid colon was performed. The operation proceeded without complication and no dissemination of disease was detected, radiotherapy was proposed as the next treatment modality. However, during the postoperative period the patient developed a strong depressive reaction with delusional features. Treatment was commenced with the antipsychotic risperidone and antidepressant fluoxetine, the patient showed some improvement with the reduction of delusional thoughts and a better mood. During further psychiatric monitoring the patient deteriorated and it was necessary to change his therapy to olanzapine antipsychotic treatment. Psychological testing has shown that the patient has above average intellectual abilities and preserved cognitive functions. However it has also shown that he is fearful of the future and has a negative attitude towards himself, with a sense of disappointment as well as a lack of motivation and faith in his chances of recovery. Discussion Patients who are confronted with the knowledge that they are suffering from a malignant disease go through various psychological phases during which they may develop serious psychiatric disorders. In the patient demonstrated, although the malignant disease was essentially treated with a positive prognosis, the patient still developed a major depressive disorder with delusional features that affected his functioning and health much more than his malignant disease.
