Normative Model for Central Augmentation Index—A Way to Identify People at Risk for Cardiovascular Disease?

Pathological atherosclerotic changes in blood vessels begin early in human life, well before the first clinical manifestations of a cardiovascular disease. Therefore, the early detection of people at risk for cardiovascular disease (CVD) is of utmost importance for its prevention. However, the existing risk assessment methods are not sufficiently precise to be applied to a general population. Experiences from studies in which the central augmentation index (cAIx), an indicator of arterial stiffness, was used together with its key determinants—age, heart rate, and height—to build sex-specific normative regression models for the Croatian population will be presented [1-2]. The cAIx, a hemodynamic parameter obtained by pulse wave analysis, was chosen as the main outcome of these models as it has been independently associated with CVD mortality and morbidity, and it is easy to measure. The models were built on the data from the population-based sample on which the strict exclusion criteria were applied in order to select the normative cohort. Observations from model building procedures, in particular the general linear modeling, will be discussed. Other methods of CVD risk stratification in the general population, and the inter-population applicability of the discussed models, will also be discussed. [1] Jerončić A et al., Sci.
