Scanning electron microscope investigation of root canal treatment

Principles of endodontic treatment require complete removal of pulp tissue, micro-organisms and debris, with the formation of a tapered endodontic space for obturation. Contemporary endodontic treatment requires thorough debridement of the root canal system with appropriate instruments in conjunction with chemical agents. The objective of this study was to investigate the root canal wall surface with reflective electron microscopy, and to evaluate the efficacy of root canal treatment. Criteria of evaluation were the quantity of smear layer and opening of dentinal tubules. The root canal walls of extracted teeth were prepared with the Canal Leader 2000 device. The preparation of 12 narrow and curved root canals of mandibular first molars was started with hand instruments up to ISO size 15 and irrigation was performed with a syringe using 2.25% NaOCl solution. The next step was to use the Canal Leader 2000 device with Universalfiles up to ISO size 30. The final preparation was done with same device, using Hedstrom files up to ISO size 50 and the stepback technique. An SEM study of the canal wall surface was undertaken by JEOL JSM 5800 electron microscope. The results demonstrated the root canal surface to have cleanliness, smooth surface without roughness or smear layer, and mostly open dentinal tubules. The results contributed to a better understanding of the importance of mechanical instrumentation and tissue dissolving by NaOCl.
