Crohn's disease of the esophagus visualized by Tc-99m antigranulocyte antibodies
A 49-year-old oman with a 20 year history of Crohn's disease reported progressive dysphagia, retrosternal chest pain, and moderate eight loss. Esophagoscopy revealed diffuse erythema with mucosal inflammation and friability. Endoscopy findings were reportedas esophagitis because focal inflammation was present. Radiologic studies showednormal small bowel transit and fistula between the rectum and vagina. To estimate the spread of the disease, a scintigraphic study with Tc-99m-labeld anti-NCA 95 antigranulocyte antibodies (AGAb) was performed (0.5 mg/BW, 250/183 ; intract murine antibody, IgG1 isotype ; CIS Biointernational, Dreieich, Germany). On the planar scans, prominent AGAb uptake in the entire esophagus was observed, suggesting Crohn's esophagitis. After treatment ith methylprednisolone and mesalamine, the patient's symptoms resolved.
