Anxiety in patients with anterior displaced temporomandibular disc
Psychological factors can affect musculoskeletal pains, the most pronounced symptom of temporomandibular disorders. The anxiety in patients with anterior disc displacement of temporomandibular joint as well as pain intensity and the success of Michigan splint treatment were compared. Disc displacement in 40 patients (mean age 35.5 years, 76% women) was diagnosed between 2001 and 2004 using Research Diagnostic Criteria for Temporomandibular Disorders Axis I and manual functional analyses. Disc displacement was confirmed by magnetic resonance imaging of the joints. Pain intensity was rated on a visual-analogue scale. The anxiety was confirmed by Spielberger’ s psychological measuring instrument. A higher level of anxiety was determined due to the fact that the mean score in State-Trait Anxiety Inventory 1 (concerning anxiety as subjective state, feeling in a period of last week, including today) was 38.43 and State-Trait Anxiety Inventory 2 (concerning anxiety as relatively stabile individual characteristic in general throughout life) was 46.10 for all examined patients. Including only patients with determined anxiety depending on age and gender resulted in 62.5% of patients with anxiety according to the State-Trait Anxiety Inventory 1 with mean score of 42.84, and 72.5% of patients with anxiety according to the State-Trait Anxiety Inventory 2 with mean score of 44.20. Statistically significant differences between patients with lower (1-4) and higher (5-10) degree of pain were rated on a visual-analogue scale for State-Trait Anxiety Inventory 2 (p=0.012). There were no statistically significant differences between anxieties measured in patients with respect to the previous pain duration, patients’ gender and the estimate of the initial therapy success (p>0.05). It is difficult to completely examine the connection between disc displacement and anxiety as the most common psychological disorder. However, patients with disc displacement experience a higher level of anxiety.
