Parental education as a confounder in the assessment of low level lead effect on cognitive function in children

The relationship between characteristic biological indicators of lead absorption (PbB, PbE, ALAD, EP), including indicators of hematological status (Hb, Hct), and tests on general cognitive development (VIQ, NIQ, IQ), visual motor integration (BG), attention and complex psychomotor reactions was studied in 275 pupils from the third and fourth grade of three elementary schools located in urban areas with different traffic conditions in Zagreb (Croatia). Additional covariates were parent and teacher ratings of child behavior, evaluated by means of an extensive questionnaire. The socio-economic background of the family was assessed through years of education for each parent that may act as proxy for parents' IQ. Lead absorption in pupils was relatively low (PbB: 70.8ą17.9 ľg/L). The anticipated difference in lead exposure with regard to the difference in traffic density in the vicinity of the schools was consistent with the difference in ALAD activity and partly with the difference in PbE level. No consistent difference between schools was found in scores for psychomotor performance. Regression analysis showed that age and parental education were the only variables significantly associated with cognitive function. The nonstandardized regression coefficients of blood lead for VIQ, NIQ and IQ were -0.016, -0.031, and -0.025, respectively, all nonsignificant. It seems that at objectively verified "low" PbB levels, the harmful influence of lead on the cognitive performance of children was smaller than that of other confounding factors, such as parental education, as demonstrated in this study.
