Comparison of breath and blood alcohol concentrations in a controlled drinking study
Introduction: The aim of this study was to test out reliability of the breath alcohol analyser in estimating factual blood alcohol concentrations following consumption of the alcohol containing beverage in healthy volunteers under controlled laboratory conditions. Materials and methods: Breath and blood alcohol concentration were determined in ten, healthy, non-smoker male volunteers after consuming red wine, containing 13 vol % of alcohol. Red wine (3 mL/kg) was consumed in the morning, after an overnight fast. Breath alcohol concentration (BrAC) and blood alcohol concentration (BAC) were determined before (baseline) and 15, 30, 45, 60, 90, 120, 150 and 240 min after beverage consumption. BAC was determined by headspace gas chromatography and BrAC was determined with Alcotest Dräger 6810 instrument. Results: The concentration- time profiles of alcohol in blood and breath following wine intake, poorly matched. Maximum values of BAC and BrAC were 0.34 g/kg and 0.61 g/kg, respectively. As determined by Bland & Altman plot, in the absorption phase the mean bias (BrAC-BAC) was 0.141 ± 0.105 (±SD) g/kg with 95% limits of agreement from -0.065 to 0.346 g/kg, whereas in the elimination phase the mean bias (BrAC-BAC) was 0.005 ± 0.030 (±SD) g/kg with 95% limits of agreement from -0.054 to 0.065 g/kg. The minimum and maximum blood alcohol elimination rates, β60, were 0.06 g/kg/h and 0.18 g/kg/h, respectively. Conclusion: The results indicate that conclusions on the factual blood alcohol concentration and related expert toxicological decision based solely on the results obtained from Dräger Alcotest breath analyser might not be correct.
