Responders and non-responders to depression screening – how do they differ?

This study investigated biases in a postal recruitment of participants for a study of behavioral treatment of depression in T2DM. The characteristics of respondents vs. non-responders, and of those who reported at least one depressive symptom vs. no depressive symptoms were compared. All available persons (N=4166) aged 18-65 living in the metropolitan area of Zagreb, Croatia, and treated for T2DM in a specialist clinic were identified using electronic patient data. Their postal addresses, sociodemographic and disease- related data (BMI, duration of DM, type of DM treatment, HbA1c, HDL, LDL, and triglycerides) were also obtained from the patient database. The participants were sent a two-item depression screening questionnaire (PHQ-2) and a response paid envelope. The characteristics of responders vs. non-responders and depressed vs. non-depressed patients were compared using one-way ANOVAs and multivariate logistic regression analyses. Univariate analyses revealed that responders were older (58.5±5.65 vs. 55.4±8.19 years, p<.001), had lower HbA1c (7.1±1.32 vs. 7.2±1.39 %, p=.031) and LDL-cholesterol (2.92±.976 vs. 3.00±1.046 mmol/l, p=.014) and higher HDL-cholesterol (1.34±.327 vs. 1.32±.322 mmol/l, p=.045). In the fully controlled multivariate model, age (OR=1.06, 95%CI=1.05- 1.08), BMI (OR=1.02, 95%CI=1.00-1.04), HbA1c (OR=.92, 95%CI=.86-.99), and LDL-cholesterol (OR=.90, 95%CI=.81-1.00) emerged as correlates of responding to the questionnaire. Those who reported depressive symptoms were younger (57.8±5.82 vs. 59.2±5.21, p<.001), had higher BMI (30.3±4.97 vs. 29.6±4.63 kg/m2, p=.008) and HbA1c levels (7.2±1.32 vs. 7.0±1.30 %, p=.014) than those who reported an absence of depressive symptoms. In the fully controlled multivariate model, reporting depressive symptoms was predicted by age (OR=.92, 95%CI=.89-.95) and BMI (OR=1.04, 95%CI=1.00-1.07). In conclusion, postal screening for depression in T2DM patients may be biased by age and disease- related characteristics.
