Hantavirus Outbreak in Croatia in 2012
Croatia is endemic for four different Hantaviruses, which also circulate in Europe: Puumala, Dobrava, Saarema and Tula virus. So far, it is confirmed that Puumala and Dobrava viruses have been causing hemorrhagic fever with renal syndrome (HFRS) in patients in Croatia. This disease is endemic throughout Croatia, except for its costal region and islands - up to 20 sporadic cases of HFRS are diagnosed annually. As in other countries, HFRS epidemics in Croatia occur every several years, and in a relatively regular manner. The last two epidemics occurred in 1995 (125 documented cases) and in 2002 (nearly 400 cases). Since January 2012, more than 60 new HFRS cases have been diagnosed. Infection was confirmed by the detection of specific IgM antibodies to Dobrava or Puumala virus, using point-of-care tests (POC® DOBRAVA-HANTAAN IgM and POC® PUUMALA IgM, Reagena Group, Tolivala, Finland), and/or by the detection of viral nucleid acids in a nested reverse transcriptase PCR, using specific primers for Dobrava or Puumala virus. The majority of cases were associated with a focus on Mount Medvednica, a prominent recreational area on the outskirts of the Croatian capital city Zagreb. The beginning of the outbreak coincided with a FIS Alpine Ski World Cup race held at Mount Medvednica in January 2012. Nonetheless, cases have been reported throughout continental Croatia, except for the southern parts. So far, Puumala virus was identified in all cases as the etiological agent. Rodents trapping at Mount Medvednica, as well as the epidemiological and ecological data collection, is in process too. However, most human cases are expected during June and July.
