Sažetak:
Introduction Awake craniotomy with special anesthesia enables brain surgery and mapping on conscious patient, also in non-eloquent brain operations. Aim Comparison of brain mapping and postoperative results in awake-brain surgery and classical neurosurgery on two patients with large frontal lobe tumor. Material and Methods Patients: male (48), and female (51). MRI: large left frontal lobe tumors. Male patient was awake, and female patient was asleep during the surgery. Psychological tests were conducted: preoperatively, postoperatively on both patients, and intraoperatively on awake patient. We performed brain mapping using monopolar, train-of- five stimuli (duration 0.5 milliseconds, interstimulus interval 4.0 milliseconds, frequency 250 Hz) for both patients, and additionally transcranial electrical stimulation (C3-Cz/C1-Cz) for asleep patient. Evoked muscles motor potentials, cricothyroid muscles long-latency response (LLR), and electrocorticographic (ECoG) after discharge pattern were recorded. Results During the mapping, the motor thresholds for hand muscles evoked response and LLR were defined for asleep patient, and ECoG after- discharge thresholds were defined for both patients. Resection was performed at sites negative to 1- to 5-mA mapping stimulation. Awake patient had paresthesias of right hand at 10 mA stimulation. During mapping in the deeper frontal brain areas, at the end of tumor resection, we detected ECoG after-discharge pattern in both patients. Awake patient developed speech hesitation and self- awareness loss, and asleep patient had LLR disappearance. The ECoG after- discharge threshold in awake patient was significantly lower. Postoperatively both patients had better language, attention, self-regulation, and social functioning status and postoperative imaging confirmed complete tumor resection. Conclusion In awake surgery, detection of acute frontal lobe dysfunctions connected with language disturbances led to better anatomical distinction of the functional areas during the operation. Direct cortical stimulation current intensity range was lower in awake surgery. Multimodal monitoring was better performed in asleep patient. The postoperative results were similar in both patients.
