Anxiety during pregnancy and postpartum
Although research on postpartum depression (PPD) has also provided some insight into anxiety, anxiety during pregnancy and postpartum has been relatively neglected. PPD differs from depression not related to childbirth in that the anxiety symptoms are more often present in PPD, and in that anxiety disorders are more prevalent than depression in the postpartum period. Anxiety during pregnancy has many adverse effects, both for maternal mental health and for the birth outcome, while also being a risk factor for PPD. As previous findings on peripartum anxiety are inconsistent in respect to prevalence, course of peripartum anxiety and comorbidity with depression, our aim was (1) to examine the course of elevated anxiety during pregnancy, immediately after childbirth, and six weeks postpartum ; (2) to establish comorbidity between postpartum anxiety and PPD ; and (3) to examine predictors of anxiety 6 weeks postpartum. A prospective study followed 272 women in the third trimester of pregnancy (T1) ; two days after childbirth (early postpartum) (T2) ; and 6 weeks postpartum (T3). At all three time-points participants filled out questionnaires on anxiety, pregnancy specific distress, stress, coping styles, social support, and depression. Obstetrical data were collected from the participants’ medical records. High anxiety levels were present in 35% of the sample during pregnancy, 17% immediately after childbirth, and 20% six weeks postpartum. Comorbidity between anxiety and PPD was 75%. Of the various demographical, obstetrical, and psychological variables, only trait and early postpartum state anxiety were significant predictors of anxiety levels 6 weeks postpartum. Elevated anxiety during pregnancy and in the postpartum period is highly prevalent. As the anxiety symptoms only partially overlap with PPD, screening for postpartum mental difficulties should include both depression and anxiety.
