Depression, Anxiety and Stress among patients with Chronic Pulmonary Diseases before and after Pulmonary Rehabilitation Program and in comparison to healthy controls

Psychological distress is common among patients with asthma and chronic obstructive pulmonary disease (COPD). Almost half of patients with chronic pulmonary diseases satisfy criteria for depression and one-third of the same group has elevated anxiety. The reason for these high percentages is presumed to be rooted in the immense quality of life disruptions due to the disease limitations in every-day life activities. Besides suffering, these psychological symptoms affect the course of the disease and overall compliance with the treatment. Therefore, medical treatment for chronic obstructive pulmonary diseases, like asthma and COPD include pulmonary rehabilitation program (PRP) that has a documented positive effect on psychological distress and overall quality of life. Pulmonary rehabilitation is an interdisciplinary, comprehensive program that consists of three week out-patient, tailor-made disease and medication education, respiratory physical therapy, physical exercise (e.g. Nordic walking), cognitive-behavioral interventions to alleviate anxiety and/or depression, supportive counselling and, if indicated - weight reduction program or smoking cessation program. Results show that after the rehabilitation COPD patients and asthmatic patients improve in all three domains of DASS-21 (depression, anxiety and stress) but they do not differ between each other, not prior or after the PRP. The results will be compared to the normative scores and discussed in the context of biopsychosocial model of understanding chronic pulmonary diseases.
